Australia Debates Public Funding for Gender Affirming Surgeries via Medicare

In early October 2026, public consultations entered their final week in Australia regarding Application 1754, currently under review by the federal Medical Services Advisory Committee (MSAC). The application proposes creating around 30 new Medicare Benefits Schedule (MBS) items for adult gender-affirming surgeries, potentially providing public subsidies for trans and gender-diverse individuals across the country. The consultation window closes on October 9, 2026.

Submitted by the Australian Society of Plastic Surgeons (ASPS) alongside the Australian Collaborative on Access to Gender Affirming Medical Services (ACA GAMS), the proposal covers chest surgery (including mastectomies), genital reconstruction, facial feminisation, and voice procedures. Proponents, including transgender advocacy organisations and surgical specialists, argue that out-of-pocket costs currently range from AUD $20,000 to over $100,000, forcing patients into debt or into seeking unregulated care overseas. While an MBS listing would not make surgeries entirely free or guarantee availability in public hospitals, it would provide substantial government rebates and allow private health insurance policies to cover hospital theater fees.

According to a statement published by rights group LGB Alliance Australia , the proposal faces significant pushback from gender-critical campaigners and clinicians. The organisation stated that the long-term clinical evidence base remains insufficient, post-operative complication rates are high, and the risk of permanent regret and detransition—particularly among cohorts who are gay, lesbian, or presenting with mental health co-morbidities—is too great. The group actively urged its supporters and detransitioners to submit formal feedback to MSAC opposing the listing before the October 9 deadline.

In April 2025, MSAC acknowledged an unmet clinical need for gender-affirming care but noted that the evidence for long-term efficacy remained highly uncertain. Following the current consultation round, MSAC is scheduled to review the revised clinical and economic assessments during its November 2026 meeting before submitting recommendations to federal Health Minister Mark Butler, who holds executive authority over MBS listings.

Medicare is Australia’s universal publicly funded health insurance scheme, funded via taxation to provide access to medical services subsidised through the Medicare Benefits Schedule (MBS). The Medical Services Advisory Committee (MSAC) is an independent statutory committee that evaluates the safety, clinical effectiveness, and cost-effectiveness of new medical procedures seeking public funding.
